PMA Membership Agreement
FAITH-BASED HOLISTIC HEALTH PRIVATE ASSOCIATION
MEMBERSHIP AGREEMENT
PRIVATE ASSOCIATION NOTICE: This organization operates as a private membership association under the rights of freedom of association, freedom of religion, and freedom of speech. We are not a public accommodation and reserve the right to accept or reject membership applications at our sole discretion.
ARTICLE I: ASSOCIATION PURPOSE AND FOUNDATION
1.1 Mission Statement
The Faith-Based Holistic Health Private Association (the “Association”) exists to provide a private forum for members who share common beliefs in faith-centered approaches to wellness, natural health practices, and holistic healing modalities rooted in spiritual principles.
1.2 Spiritual Foundation
This Association operates under the belief that true healing encompasses spirit, mind, and body, rooted in biblical teaching and the recognition that Jesus Christ is the divine source of all healing and wellness. We believe that God’s Word provides guidance for health and wholeness, and that Jesus Christ, as our Lord and Savior, is the ultimate healer. Members acknowledge and respect this foundational Christian principle.
1.3 Educational Purpose
The Association serves as an educational and informational resource, facilitating the sharing of knowledge, experiences, and resources related to faith-based holistic health practices among consenting adult members.
ARTICLE II: MEMBERSHIP REQUIREMENTS AND PROCESS
2.1 Eligibility Requirements
To become a member, an individual must:
- Be at least 18 years of age
- Complete and submit a membership application
- Pay required membership fees
- Agree to uphold the Association’s values and principles
- Acknowledge understanding of the private nature of this Association
- Complete membership interview
2.2 Application Process
Membership applications will be reviewed by the Association’s leadership. The Association reserves the absolute right to accept or reject any application for any reason, without explanation.
2.3 Membership Types
- Health Fellowship Membership: Access to educational materials, workshops, and general Association activities. Care practices outlined in your package level.
- National Membership: Acceptance and coverage within the association.; Members ability to apply for accreditation through the Badge system. Member listing and business directory listing online as well as inside the building.
- Business Membership: Acceptance and coverage within the association.; Members ability to apply for accreditation through the Badge system. Member listing and business directory listing online as well as inside the building. Use of space within the facilities. Podcast and other speaking opportunities.
2.4 Membership Duration and Renewal
Memberships are valid for one (1) year from the date of acceptance and must be renewed annually to maintain active status.
ARTICLE III: MEMBER RIGHTS AND RESPONSIBILITIES
3.1 Member Rights
Members in good standing have the right to:
- Access Association educational materials and resources
- Participate in Association activities and events
- Receive information about holistic health practices
- Engage with other members in a respectful, faith-centered environment
- Privacy regarding their membership and participation
3.2 Member Responsibilities
Members agree to:
- Conduct themselves in a manner consistent with Christian principles and Association values
- Respect the confidentiality of other members and Association activities
- Take personal responsibility for all health decisions
- Seek appropriate professional medical care when needed
- Not hold the Association liable for any health outcomes
- Comply with all applicable laws and regulations
ARTICLE IV: SERVICES AND OFFERINGS
4.1 Educational Services
The Association may provide:
- Educational workshops on faith-based health principles
- Resource libraries and materials
- Guest speaker presentations
- Study groups and discussion forums
- Wellness coaching and guidance
4.2 Private Nature of Services
All services provided are:
- Offered within the private membership framework
- Educational and informational in nature
- Not intended as medical diagnosis or treatment
- Shared among consenting adults in a private setting
4.3 Member Consultations
Any consultations provided are:
- Educational discussions between members
- Based on traditional, natural, and faith-based approaches
- Not medical advice or treatment
- Conducted with full member consent and understanding
ARTICLE V: DISCLAIMERS AND LIMITATIONS
5.1 Medical Disclaimer
IMPORTANT NOTICE: The Association and its members do not practice medicine or provide medical advice without proper licensing and approval. All information from non-licensed professionals shared is for educational purposes only. Members are encouraged to consult with licensed healthcare providers for medical conditions and before making significant health decisions.
5.2 No Professional Relationship
No doctor-patient, practitioner-client, or other professional relationship is created through Association membership or participation in Association activities.
5.3 Personal Responsibility
Members acknowledge that they are solely responsible for their health decisions and outcomes.
ARTICLE VI: LIABILITY AND INDEMNIFICATION
6.1 Limitation of Liability
Members agree that the Association, its officers, directors, members, and volunteers shall not be liable for any injury, illness, adverse reaction, or other harm that may result from participation in Association activities or use of information provided.
6.2 Assumption of Risk
Members voluntarily assume all risks associated with their participation in Association activities and use of information provided.
6.3 Indemnification
Members agree to indemnify and hold harmless the Association and its representatives from any claims, damages, or expenses arising from their membership or participation in Association activities.
ARTICLE VII: PRIVACY AND CONFIDENTIALITY
7.1 Member Privacy
The Association respects member privacy and will not disclose membership information to third parties except as required by law or with explicit member consent.
7.2 Confidentiality Expectations
Members agree to maintain confidentiality regarding:
- Other members’ identities and personal information
- Discussions and activities within the Association
- Private consultations and educational sessions
7.3 Record Keeping
The Association maintains minimal records necessary for operation and will protect member information in accordance with applicable privacy laws.
ARTICLE VIII: FEES AND FINANCIAL TERMS
8.1 Membership Fees
- Annual membership fees are due upon application and renewal
- Fee schedules are established by the Association and may be modified with notice
- Fees are non-refundable except at the sole discretion of the Association
8.2 Additional Fees
Additional fees may apply for special events, workshops, or enhanced services, as determined by the Association.
8.3 Fee Modifications
The Association reserves the right to modify fee structures with thirty (30) days notice to members.
ARTICLE IX: TERMINATION AND DISCIPLINE
9.1 Voluntary Termination
Members may terminate their membership at any time by providing written notice to the Association.
9.2 Involuntary Termination
The Association may terminate membership for:
- Violation of this agreement
- Conduct incompatible with Association values
- Failure to pay required fees
- Breach of confidentiality
- Any reason deemed appropriate by Association leadership
9.3 Effect of Termination
Upon termination, all member rights cease immediately, and the member must return any Association materials and cease representing any connection to the Association.
ARTICLE X: GENERAL PROVISIONS
10.1 Governing Law
This agreement shall be governed by the laws of [Texas], without regard to conflict of law principles.
10.2 Entire Agreement
This agreement constitutes the entire agreement between the member and the Association and supersedes all prior negotiations, representations, or agreements.
10.3 Amendments
This agreement may only be amended by written document approved by Association leadership, with 30 day notice provided to members.
10.4 Severability
If any provision of this agreement is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.
10.5 Private Association Status
This Association operates as a private membership organization exercising rights of freedom of association, freedom of religion, and freedom of speech. We are not subject to licensing requirements applicable to public businesses or healthcare providers.
MEMBER ACKNOWLEDGMENT AND SIGNATURE
By signing below, I acknowledge that I have read, understood, and agree to be bound by all terms and conditions of this Membership Agreement. I understand that this Association operates as a private membership organization focused on education and fellowship around faith-based holistic health principles.
I acknowledge that:
- No medical advice or treatment is provided by the Association
- I am responsible for my own health decisions
- I assume all risks associated with my participation
- I will maintain confidentiality regarding Association activities and other members
- I understand the private nature of this organization

This agreement is effective as of the date signed and governs the relationship between the member and the Faith-Based Holistic Health Private Association known as Living Holistic Health Fellowship.
Upon notice of the leadership board of Living Holistic Health Fellowship, the Private Membership Association, the new member will receive the Signature Stamp of acceptance to begin conducting business within the Association.
